Office Guidelines

    Request of appointment

  • Our offices are open between 8:30A.M. and 5 P.M. Monday thru Friday. Please see the specific office location information to confirm the hours of operation.
  • Patients are seen by appointment only.

  • New insurance

  • Please call 301-856-5860 to verify if your new insurance is contracted with and if any change in your usual appointment is needed.

  • You were involved in an auto accident

  • Please when you make your appointment bring your auto insurance information: claim number, PIP information, state in which accident occurs, if you were a tiers involve, what is goint to happen when the PIP is exhausted.

  • You were involved in a work injury

  • We will need the claim number and your work compensation name and phone number.
  • We will need this 48hrs prior to the appointment so we can request an authorization from your adjustor for the visit.

  • Referrals

  • If your insurance requires a referral please bring it with you.
  • If your insurance requires an electronic referral please be sure to get the referral number from your primary care doctor. Please note that you will need the referral information for your appointment to be able to be seen.

  • Copayment

  • All copayment are due at the time of service.
  • The payment can be made in cash, or credit card. At your insurance request we cannot waive or bill you for your copay. We accept checks for bill payment only.
  • A 35$ charge will be added to any returned payment.

  • Payment method

  • You will be able to pay your bill at the front desk in any of our offices.

  • Medication refills

  • Prescriptions and refills will only be issued with an appointment.
  • To ensure that your medication needs are met in a timely manne,r we request you to set up your next appointment at checkout or call at least 2 weeks prior to the date your medication is scheduled to run out.

  • Form processing:

  • For forms there will be a 5$ fee for each page completed by the doctor, with the exception FMLA forms, the fee regardless of the pages will be $35.
  • The fee for medical records will be 0.73cts per pages. The turn around time for both of these request will be 48 to 72 hours.

  • Running late/cancellation and rescheduling policies

  • Appointments should be cancelled within 24 hrs to allow other patients an opportunity to be seen.
  • Any patients that fails to show up for their scheduled appointment will be charged a no show fee of 25$ for an office visit and 200$ for a procedure appointment.
  • We will be unable to schedule future appointment for patients having 3 missed appointment and/or cancellation without the appropriate notice, particularly if we feel that these missed appointments are adversely affecting our intervention/treatment plan.
  • Please call if you are running late. We will allow you to arrive 10-15 minutes past your appointment time. Patients arriving 20 minutes late may be asked to reschedule their appointment.

  • Non Covered Services (prolotherapy, Platlet rick plasma kit, etc..) must be paid for at the time of service


    No Food and drinks are allowed in the office. You are discouraged from bringing family members specially children to the office due to limited space.
